An innovative solution for ensuring perfect chin implant placement
We present the latest surgical approach developed by Dr. William J. Binder, MD, FACS, and enthusiastically discussed by Dr. Barry L. Eppley, MD, DMD. This innovation marks a breakthrough in aesthetic surgery, focusing on improved precision and safety in chin implant placement. Photos are courtesy of Dr. Binder.
Until now, the only widely recognized limitation in creating a subperiosteal pocket for a chin implant was avoiding the mental nerve. However, with this new method, that concern becomes a thing of the past.
Proper placement of the implant along the inferolateral border of the chin and jawline ensures safe and optimal chin projection without the risk of damaging the mental nerve.

What does this innovation bring?
Previously, surgeons often created the pocket too high on the chin, which could cause the implant wings to press on the mental nerve or even be placed above it. The main challenge was the persistent attachments of osteocutaneous ligaments along the sides of the chin.
Instead of avoiding these attachments by dissecting above them, the new technique recommends their release. This allows for more precise and secure implant placement.

Advantages of the new method
- Greater tissue mobilization: Incising the anterior "mental" chin ligamentous attachments and elevating the more lateral mandibulocutaneous ligamentous attachments at the subperiosteal level enables greater flexibility in implant placement.
- Long-lasting results: The periosteum does not reattach after the procedure, so the mobilized soft tissue remains elevated, contributing to longer-lasting stability and aesthetics of the implant.
